How to Verify Your Coverage

To avoid unexpected costs, we encourage all patients to contact their insurance provider prior to their visit. When you call the number on the back of your insurance card, ask these three questions:

  • “Is Western Surgical Group (and my specific surgeon) in-network for my plan?”
  • “Does my plan require a referral or prior authorization?” (If yes, you must obtain this from your Primary Care Physician before your visit).
  • “What is my deductible and specialist co-pay?”

Referrals & Prior Authorizations: If your insurance requires a referral or prior authorization, you must obtain this from your Primary Care Physician before your visit. Our team will assist you in coordinating this with your primary care physician to ensure all necessary documentation is in place.

Financial Policy

  • Co-pays: Please be prepared to pay your designated co-pay or co-insurance at the time of service.
  • Referrals: If your insurance requires an authorization, please ensure this is secured by your PCP and brought with you to your appointment. Without a required referral, your appointment may need to be rescheduled.
